How to Refer a Client

Our referral process is simple, confidential, and designed for fast response times.

Complete the secure Referral Submission Form online or download a PDF to fax/email directly to our admissions department.

Include the client’s name, contact details, presenting concerns, and any supporting documentation (such as assessments or discharge summaries).

A Vital Health intake coordinator will contact the client within 24 hours to schedule an assessment and verify eligibility and insurance coverage.

We maintain open communication with referring providers (with consent) to ensure continuity of care and progress updates.
