Vital Health case management program provides goal-oriented and individualized supports focusing on improved self-sufficiency for the persons served through assessment, planning, linkage, advocacy, coordination, outreach efforts, secure housing and explore employment activities and monitoring activities. Vial Health QMHS have the knowledge/resources with healthcare, social services, employment, housing, recreational opportunities, and other services systems available in the community. SUD/Case Management will work with adults and adolescents to provide the best care and access to the persons served and is preferred by the persons served; will vary with respect to hours, type and intensity of services, depending on the changing needs of the individual. The persons served are linked to services and resources to achieve objectives as identified in their person-centered plan (ITP). Personnel providing services have a working knowledge of the: (a) services that are appropriate for the needs of the persons served and (b) support systems that are relevant to the lives of the persons served.
